The News Minute | June 30, 2014 | 11.43 pm ISTPreliminary inquiries by the Petroleum Ministry have found serious lapses on the part of Gas Authority of India Limited, which led to the fire last Friday killing 19 people in Andhra Pradesh.A report published by the Indian Express said that GAIL suspended two officials - General Manager Pankaj Patel and Deputy General Manager Rakesh Kumar. The fire occurred in Nagaram village of East Godavari district. The fire broke out after a tea vendor lit a stove near the leak and devoured everything in a half-kilometre radius. The Indian Express reported: “Preliminary findings by the government team showed that the Tatipaka-Kondapalli pipeline had corroded, for the fourth time this year, and the leak was caused by condensate and water in the gas supplied by ONGC, which should have been completely stripped by GAIL at their Nagaram facility.”
